We look forward to meeting you on your first visit to our office. During this time, we will help you get acquainted with our office and learn about orthodontic treatment. We understand that each patient’s orthodontic treatment needs are different, and we are dedicated to give you the personal attention you deserve. Your first visit with Dr. Glovsky is complimentary and will consist of digital radiographs and a discussion of potential treatment options. At this visit we will provide a treatment plan and review financial arrangements. The initial consult will address your orthodontic needs to determine if this is the proper time to begin treatment. Dr. Glovsky encourages questions from both the patient and parent.
Please download the new patient forms below, fill them out and bring them with you to your first visit.
We are open to serve you, answer questions or schedule an appointment during the following hours:
Monday – Wednesday: 8 a.m. – 5 p.m.
Thursday: 9 a.m. – 4 p.m.
Your scheduled appointment time has been reserved specifically for you. We request 24-hours notice if you need to cancel or reschedule your appointment.
We believe that financial obstacles should never stand in the way of quality orthodontic treatment! Our office offers a low down payment treatment. We provide our patients with a variety of payment options to suit their unique financial needs. For your convenience, we offer comfortable in-house payment plans with no interest. Payments may be remitted by cash, check, Visa, MasterCard, Discover and American Express.
If your insurance covers orthodontic treatment, you will receive the benefit of reduced personal costs. Many insurances have a lifetime orthodontic benefit that is separate from your regular dental benefit. Remember that insurance policies vary, and we will review your insurance to determine the appropriate course of action. Once treatment has started, we will file your claims.
To help us with your insurance filing, please complete the insurance portion of the patient information form. Orthodontic insurance will be filed by our office but please understand the entire balance is the patients responsibility. If, during the course of treatment, your benefits change or are cancelled the patient (responsible party) is responsible for the entire balance owed.
